In January 2026, Switzerland introduced the new outpatient tariff structure TARDOC for services reimbursed under compulsory health insurance (KVG/LAMal), replacing the long-standing TARMED tariff used since 2004. TARDOC aims to modernize the remuneration of outpatient services by better reflecting the complexity, time and resource use of medical activities.
It was jointly developed by medical associations and health insurers and approved by the Federal Council after several years of negotiation and revision. The structure includes updated service definitions and relative value units intended to improve transparency and reduce distortions in physician remuneration. It also aligns more closely with the growing shift of services from inpatient to outpatient care.
The introduction of TARDOC is accompanied by transitional monitoring to ensure system-wide cost neutrality during the initial rollout. The tariff structure remains subject to iterative refinement as empirical data and operational experience with the new system accrue.
